Camp Abilities at CTEBVI 2009

Important Note: Please open and read the Camp Abilities Flyer before filling out all paperwork.  There are restrictions on this event. It is not open to the public.

All three files must be printed and completed by the responsible parent, and sent with a check of $60 for EACH child participating. Please make your check payable to CTEBVI and indicate "Camp Abilities" in the memo line of the check.

Please find attached the registration forms for Camp Abilities. 

There are three attachments because there are a lot of forms/waivers to be signed.  So, each form must be opened and completed by the parents.  This is really important.
